INSTRUCTIONS FOR OBTAINING A MARRIAGE LICENSE:

(A Marriage License is a legal document and all information you give is final.)

1. Carefully complete the Information for Marriage License form. The Groom

and the Bride must complete their own forms. Please print clearly.

Acceptable Government-issued identification with a birth date may be

required to prove your age.

A. Name:

Print clearly your full name, including your first, middle and last

name. Brides will indicate their current last name, not the new name

you will have after you are married.

B. Residence:

Print clearly the city or town and state where you currently live. If

your usual residence is not in the United States, enter “N/A” for

Resident State, then enter your Resident Country.

C. Date of Birth:

When were you born? Provide month, day and year. What is your

age?

D. State of Birth:

State where you were born. (If not in the United States enter “N/A”

for Birth State, then enter the country where you were born.)

E. Social Security Number:

Enter your Social Security Number.

F. Marital Status:

Check the appropriate box that pertains to your Marital Status.

G. Number of this Marriage:

Total Number of times you have been married, including this one.

H. Date Divorce or Annulment Final:

What date was your divorce or annulment final – month, day and

year? All divorces must be final in the state or country where they

were granted.

I. Widowed When:

If your previous marriage ended due to death of spouse, enter the

date you were widowed.

J. Where Divorced, Annulled, or Widowed:

City or town and state that the divorce or annulment was granted in.

If previous marriage ended due to death of spouse, enter the city or

town and state where spouse died. If not in the United States, enter

“N/A” for State, then enter the country where this occurred.

K. Mother’s Name:

Mother’s first name, middle name (optional), and maiden name,

(maiden name is her last name at birth).

L. Mother’s State of Birth:

State where your mother was born. ( If out of the United States,

enter “N/A” for Birth State, then the country where she was born.)

M. Father’s Name:

Father’s first name, middle name (optional) and last name.

N. Father’s State of Birth:

State where your father was born. ( If not in the United States, enter

“N/A” for Birth State, then the country where he was born.)

O. Mailing Address:

Enter your complete mailing address.

2. In Nevada, the Marriage License and Marriage Certificate will reflect your

names as submitted. The Marriage Bureau does not enter the new last name

of the bride or make name changes. You will need to check with the

Department of Motor Vehicles, Social Security or legal offices in the state

or country you reside in for further instructions if you wish to change your

name to your new married name.

3. Please check the information on the Marriage License carefully. All

information you submitted and the clerk has entered on the Marriage

License should be correct and error free. No corrections may be made after

the ceremony.

4. Please read the oath. You will be swearing or affirming all information is

true and correct with your signatures at the bottom of the Marriage License.

5. In order to have a legal marriage, the Groom and Bride must first purchase

the Marriage License, then they must go through a marriage ceremony. The

ceremony must be performed in the State of Nevada by any person licensed

or authorized to perform ceremonies in Nevada. Clerks in the Marriage

License Bureau are barred from recommending or giving advice as to where

to have a ceremony. The ceremony may be performed in any Wedding

Chapel, Church or by a Civil Marriage Commissioner. Check the yellow

pages under Weddings in the phone book.

6. Certified Copies of your Marriage License may be purchased at the time of

issuance of the Marriage License, or you may mail your request and fees at

a later date. To request a copy of your Marriage License, contact the

County Clerk.

8. Some countries may require a certified copy of your Marriage Certificate

and an Apostille from the Nevada Secretary of State to finalize the

marriage in your country. For further instructions and fee, check with the

After BOTH THE BRIDE AND GROOM have completed a Marriage License

application, they will need to come together to the regional Government Center located at

101 Civic Way in Laughlin Nevada if getting married in Laughlin, or to the Marriage Services Division at

200 South Third Street, LasVegas, Nevada on the first floor to obtain the license.

The fee for a marriage license is $55.00. For more information about obtaining
